PTSD Self-Test

Below is a list of twenty symptoms and problems that people sometimes have in response to traumatic life experiences.

This questionnaire is designed to determine whether you demonstrate symptoms similar to those of adults with Post-traumatic Stress Disorder (PTSD) and is intended to be used as a screening tool; it is not diagnostic. If you answer 'Moderately', 'Quite a bit', or 'Extremely' to a significant number of these items, consult your family doctor and/or a psychologist who is trained and experienced in diagnosing and treating PTSD.

An accurate diagnosis can only be made through clinical evaluation by a trained mental health professional.

Adapted with permission, from PCL-5 Weathers, Litz, Keane, Palmieri, Marx, & Schnurr – National Centre for PTSD. This is a government document in the public domain.

Questions

Please read each item carefully and choose the response that indicates how much you have been bothered by that problem in the past month:

According to your responses, you have symptoms highly consistent with PTSD and further investigation is warranted.
According to your responses, you do not appear to have symptoms consistent with the scope and severity for a PTSD diagnosis.
